Understanding Erectile Dysfunction

  Erectile dysfunction is defined as the persistent difficulty achieving or maintaining an erection sufficient for sexual activity. While occasional difficulty is common, ongoing symptoms may reflect underlying physiologic contributors such as:

  • Vascular health and blood flow
  • Hormonal balance
  • Neurologic signaling
  • Metabolic health
  • Medication effects or chronic disease

  ED is frequently associated with conditions such as insulin resistance, obesity, hypertension, and low testosterone, and may serve as an early indicator of broader cardiometabolic risk.

  Effective treatment begins with identifying contributing factors rather than relying on symptom-only management.

Therapeutic Options Used at Apex

Treatment is individualized. Not all patients are candidates for every therapy.


-Oral Therapy

Tadalafil and Sildenafil may be prescribed to support erectile function through vascular mechanisms. Selection and dosing are based on response, tolerability, and medical history.


-Adjunctive & Central Therapies

PT-141 (Bremelanotide) may be considered in select cases where central arousal signaling is a contributing factor.

In certain situations, compounded Oxytocin / Tadalafil / Apomorphine troches may be used as adjunctive therapy when clinically appropriate.


-Injection Therapy

For patients who do not respond adequately to oral or adjunctive options, intracavernosal injection therapy (TriMix) may be considered.

TriMix is a compounded injectable medication designed to directly support penile blood flow. Use requires careful patient selection, individualized dosing, and medical supervision.
